The average EMS agency has no idea what a single ambulance transport actually costs them. Ask a fire chief or EMS director and you will get a number — usually somewhere between $400 and $800. Press them on how they calculated it, and the answer is almost always the same: total budget divided by total transports. That top-level division hides the real cost drivers and makes it impossible to identify where money is being wasted.

The Full Cost Model: What Goes Into Every Transport

A true cost-per-transport analysis breaks expenses into six categories. Each one contributes a different share depending on your service model, call volume, and geography.

1. Staffing Costs (55-70% of Total)

Labor is the overwhelming cost driver in EMS. A typical ALS (Advanced Life Support) unit requires a minimum of two crew members per shift: a paramedic and an EMT. Here is what that costs:

The average agency has a 60-75% transport rate (the rest are refusals, cancelled calls, or standby). The Revenue Side: What You Actually Collect

Knowing your cost is only half the equation. The other half is what you collect per transport — and this is where most EMS agencies discover they are operating at a loss.

Medicare/Medicaid Reimbursement

Medicare pays a base rate plus mileage. In 2026, the national average Medicare allowable for an ALS-1 emergency transport is approximately $500-$530 base plus $8.50-$9.00 per loaded mile. Medicaid rates are significantly lower — often 50-70% of Medicare rates, varying wildly by state. Some states pay as little as $120 for a BLS transport.

Commercial Insurance

Commercial payers typically reimburse at 80-150% of Medicare rates. However, balance billing restrictions in many states mean you cannot bill the patient for the difference between your charges and the insurance payment — even when that difference is substantial.

Effective Collection Rate

Across all payer types, the average EMS agency collects 55-65% of billed charges. If you bill $1,200 per ALS transport, you are collecting $660-$780 on average. After subtracting the cost per billable transport ($280-$420), the margin per transport is thin — often $250-$500 — and many agencies operate at break-even or a loss.

Break-Even Analysis

The break-even formula for an ambulance unit is straightforward:

Break-even transports = Annual fixed costs / (Revenue per transport - Variable cost per transport)

For a unit with $550,000 in annual fixed costs (staffing, vehicle, overhead), $80 in variable costs per transport (supplies, fuel), and $650 in average net revenue per billable transport:

$550,000 / ($650 - $80) = 965 billable transports per year to break even. At a 70% transport rate, that means roughly 1,380 total responses — or about 3.8 calls per day. Many rural agencies fall below this threshold, which is why subsidies and tax levies are so common in EMS.
